Family Income Inquiry Form

2023-2024 Family Income Inquiry Form (previously known as School Meals Form)

This form must be completed each school year.

Please complete a 2023-2024 Family Income Inquiry Form online.

You can submit one form for all children in the household attending school in grades Pre-K through High School.

We ask that you support us in completing this form; ensuring our school receives additional funding. 

Note: If families qualify according to the information submitted on the Family Income Inquiry Form, their child will be entitled to:

• College Board and ACT fee waivers
• College application fee waivers
• A waiver of Advanced Placement (AP) exam fees (if they are waived)

• The Zip Code for The Brooklyn School for Social Justice is 11237.
• School District is New York City Schools.
• The DBN is 32k549 (previously known as The Bushwick School for Social Justice). 

Completing your form online is quick and easy. Forms submitted online are processed quickly and you will receive immediate confirmation that your form was submitted.  Please retain the confirmation information.  If you do not wish to file your application online, you can request a paper application from the school, via your Parent Coordinator.  Please note that it may take longer to process your application if you file via paper.​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​
